First Visit

We encourage all our new patients to complete our New Patient Form prior to arrival. If you are unable to complete the form, please arrive about 15 minutes earlier to give yourself enough time to fill out a New Patient Form.

Direct Billing to Insurance

We offer direct billing for all private insurance plans, government programs of NIHB, ODSP and HSO. We can submit your insurance claim electronically. Our team is able to help you understand your insurance plan better. We will send estimates to your insurance company to get better financial preparation before starting any major treatment.
Please bring you insurance plan information to your appointment.

Payment Methods

Any co-pays or amounts not covered by insurance are your responsibility, and the amount must be paid at the time of your appointment.

We accept payment by cash, debit, e-transfer or most major credit cards.

Languages

Our office offers dental services in English. However, our team can help with interpretation of Mandarin and Spanish.
